1. What is B Form?
B-Form is known by different names like Form- B, Bay-Form (بے فارم، ب فارم) and CRC (Child Registration Certificate). All are same documents.
B-Form is issued by NADRA for all
the children less than 18 years of age in a family. The names of all children
of a family who are less than 18 years of age are incorporated in one B
Form. Besides, their own names, their father’s name, mother’s name, Identity
Number, Date of birth, Place of birth/ District of Birth is also mentioned in B
Form.
B Form
establishes the identity of each child mentioned in B Form and can be
produced as an identity in place of ID Card.
2. Purposes of B-Form
A Form B being an identity document can
be used for various purposes like for obtaining Domicile Certificate, taking admission
in school, appearing in examination s, issuance
of guardianship certificate from court, in support of Visa Application form etc.
Be noted, in some places, Juvenile Card may be requested to be produced instead
of B Form because in such place B form may not be acceptable.
3. Documents and other requirements for issuance of Form B?
1.
Anyone from parents should personally visit NADRA
Centre
Or
In
case of guardian, the guardian should personally appear in NADRA office along
with guardianship certificate and other relevant proof of appointment of
guardian.
2.
CNIC of parent or the Guardian, whatever the case may
be.
3.
Marriage Certificate of parents (not mentioned on
official website of NADRA but I recommend to take it along)
4.
Birth Certificate (verified) issued by Union Council
or Cantonment Board etc.
5.
Biometric Verification of one of the parents or
guardian
Or
Attestation of form by
Attester
4. How to get Form B from NADRA?
After arranging the documents, visit your nearest NADRA
office along with documents. The parents or any of the parents or Guardian should
accompany the child to NADRA office.
NADRA will make relevant entries and may take biometrics and
will process the application for B Form.
